Help needed to build community outreach database

Your help is needed to develop a central directory for MUSC community outreach efforts. Collecting information for this type of directory is important to help us accomplish the following goals:

  • Create a database for community outreach programs and services that can be updated regularly and accessed by all.
  • Create an MUSC community calendar that can be used to promote programs and services via television, radio, newspaper, Internet and MUSC publications.
  • Maintain up-to-date information for our referral call center. The call center handles local and nationwide toll-free consumer calls through the MUSC Health Connection number (792-1414 or 1-800-424-MUSC). Health Connection provides access and referral to MUSC health care services, as well as health and programming information. Calls are entered into a database according to their request, providing valuable market research tracking data. If we do not know about your outreach efforts, we may be unable to communicate important facts to callers.

You can help us collect this needed information by letting us know what you and your colleagues are doing in the community. Share this information with others in your area who are involved with community outreach programs. Because we want to maintain a timely and accurate database, we will request future updates on a regular basis. For information, call 792-8258.

Community Outreach Activity Questionnaire

Please provide the information requested below and return it to MUSC Marketing Services, 152 Cannon St., or fax it to 792-9568. Make additional copies of this form for each program you submit. The deadline is Dec. 15. Please direct questions to Lisa Giles at 792-8258.

Examples of information needed include:

  • Health screenings offered such as cholesterol, blood pressure, vision, etc.
  • Departments and services willing to participate in health fairs, provide speakers for community groups, etc.
  • Support groups or regular classes offered.
  • Any MUSC program or service that reaches out to the community for the purpose of medical service or health education. This includes research, academic, clinical and volunteer programs.
