MUSC uses electronic database for home care referrals

In an effort to promote a seamless transition from the hospital setting to the home care setting MUSC utilizes an electronic home care referral program that streamlines the referral process. Known by the acronym, HARP, the Home Care Agency Referral Program is a six step point and click online computer application that allows persons involved in the discharge planning process to alert home care coordinators of potential discharge needs early during a patient’s hospitalization. 

While designed to enhance the continuum of care, HARP is also intended to be in compliance with Federal law mandating that patients have the right to select their own home care provider. For each patient requiring a home care referral a list of home care providers (by county of residence) may be generated from the program. The list is then offered to each patient allowing the patient/family the option of selecting a home care provider.

Case managers, nurses and social workers initiate HARP referrals. As the referral is made an e-mail and an electronic page is sent to the home care coordinator(s). The home care coordinator(s) respond by interacting with members of the care team, patient/family, contacting the selected agency, faxing the appropriate clinical information and documenting on the patient database that the home care arrangements have been completed. This function is an integral part of the discharge planning process and promotes the timely movement of patients along the care continuum to the appropriate care setting. 

All newly hired registered nurses and social workers for UMA, MUHA, and IOP are instructed in the use of HARP during computer orientation class. 

Additionally, a class is now offered on the third Friday of each month for current employees who would like a review of the program.
