Mandatory direct deposit for all
employees
Beginning Jan. 1, 2006, mandatory direct deposit of payroll checks will
apply to all Medical University of South Carolina employees, including
part-time, temporary, and student employees. Individuals not currently
utilizing direct deposit must make arrangements for this payment method.
Employees who need to arrange for direct deposit should complete these
steps:
- Set up a bank account, if you do not already have one.
Several Charleston banks provide free checking services to MUSC
employees. Employees should review these local banks agreements for
providing no cost direct deposit services.
- Fill out the Medical University of South Carolina
Authorization Agreement for Direct Deposit. This form may be accessed
at www.musc.edu/fanda/faforms/
- Then scroll down to payroll and the first form is the
Direct Deposit Authorization Agreement.
- Attach a voided check to complete this form.
- Fax or deliver form to the Payroll Office, Room 702
